CCSI is the Coordinated Point of Entry/ Senior Information Services provider for McLean County.  SIS staff help older adults, people with disabilities, grandparents or relatives raising children, and their care partners find needed services and resources in the community to maintain their independence. Individuals, families and friends interested in learning more about home and community-based services and supports for those ages 60 and over are encouraged to contact the Care Coordination Unit (CCU) in your area (listed to the right) through the Community Care Program (CCP). Once a referral is made a Care Coordinator will make a home visit and assist the participant in determining eligibility for CCP and in developing a person-centered plan of care. CCP core services include homecare aides, adult day services, emergency home response services and automated medication dispensers. In addition to CCP services, the CCU can link older adults to community-based services offered by the local Area Agency on Aging.

The Caregiver Advisory program provides help to an adult family member, or another individual, who is an informal provider of in-home care to an older individual in the form of information, assistance in gaining access to services, individual counseling/consultation, and support services to help caregivers and grandparents raising grandchildren cope with their caregiving roles and/or develop and strengthen capacities for more adequate social and personal adjustments
