Microsoft Forms

Microsoft Forms is available to all Illinois State faculty, staff, a​nd students through Office 365 Online. You can create surveys, quizzes, and polls, and easily see results as they come in. You can invite others to respond to it using any web browser, even on mobile devices. Surveys can be shared with participants beyond Illinois State.

Built-in analytics allow you to evaluate responses in real-time, allowing you to use forms to poll live audiences during a presentation or lecture Form data, such as quiz results, can be easily exported to Excel for additional analysis or grading. To create or edit a survey or quiz in Microsoft Forms, a valid Illinois State ULID and password is required at log in.

Step 1: Sign in with a valid Illinois State ULID and password

Step 2: Create a new survey​ or create a new quiz

Step 3: Adjust settings for your survey or quiz

Step 4: Send your survey or quiz to others and collect responses

Step 5: Check the results of your survey or quiz
